I hereby give an undertaking and authority to Management of the aforementioned institution to process and effect a deduction of two percent (2%) of my basic monthly salary as Union Membership Subscription Fees. This is in accordance with the Constitution of ZUFIAW and Section 23 of the Industrial and Labour Relations Act of the Laws of Zambia. This order shall remain in force until written notice is given by the undersigned.
